How to Donate to the USHJA Foundation
The USHJA Foundation offers donation levels to meet the needs of any level of support.
You can help us make a difference in our sport by supporting the USHJA Foundation.

to donate knowing that .90 cents on the dollar goes directly to programs and development.
Donations can also be made my submitting the donation form(PDF).
Supporter $10 to $49 — Foundation pen, pin and notepad and USHJA Foundation window decal.
Contributor $50- Up To $999 —USHJA Foundation, pen, pin, notepad, window decal, baseball cap.
Patron $1000-Up to $7499 — If donated in 2010 donor becomes a member of the USHJA Founders Circle and
name on plaque in the USHJA headquarters. You receive a USHJA Foundation baseball cap, pen, pin, window decal,
notepad and tote bag.
Sustainer $7500-Up to $19,999 — If donated in 2010 donor becomes a member of the USHJA Founders
Circle and name on plaque in the USHJA headquarters. Name and logo/link if applicable will be placed on the
USHJA website, annual listing in the USHJA magazine and name on plaque in the USHJA headquarters. You receive a
USHJA Foundation baseball cap, pen, pin, window decal, tote bag, leather journal and fleece vest.
Benefactor $20,000 up to $49,999 — If donated in 2010 donor becomes a member of the USHJA Founders Circle.
Name and logo/link if applicable will be placed on the USHJA website, annual listing in the USHJA magazine and name
on plaque in the USHJA headquarters. You receive a USHJA Foundation baseball cap, pen, pin, window decal, notepad
and tote bag, USHJA fleece vest and USHJA tie or scarf.2 Annual Meeting Registrations for year in which pledge is made.
President’s Circle $50,000 and above
The USHJA Foundation’s President’s Circle is a status reserved for donors
who’s passion to support our industry goes beyond a love for the sport and

Founder’s Circle
An exciting way you can support these programs and leave a lasting impression is to become a
member of the USHJA Foundation Founders Circle. Anyone that donates $1000 or more through the
Foundation before December 31, 2010 will become a member and have their name on a plaque in the
USHJA building. There is more information and pledge cards on our table by registration or you
can ask any member of the Foundation Board about becoming a Founders Circle Member.
